Online Application Guidance Videos

We've created a series of short videos to help our members apply for accreditation using our online form. Our videos provide detailed guidance for each step of the application process.

The online application form doesn't work with Apple devices. If you're using an Apple device, please download and complete the Word version of the application form.

Our online form works best with either Google Chrome or Microsoft Edge. We recommend updating your browser to the latest version before you start your application.

 

Getting started

To start your application, click on the login to purchase button. If you're already logged in, click on apply.

 

Current practice details

Please provide details of your current practice only. This section shouldn’t be used to provide details of your previous practice settings.

 

Practice log

Please submit at least 450 supervised practice hours over a minimum of three and a maximum of six years of practice. All practice submitted must be covered by at least 1.5 hours of supervision per month.

 

Supervision details

For each year of practice you have added to the practice log, and current practice details, you need to allocate the details of your supervision contracts to each year and as well as your current practice setting or settings.

 

Diary of current practice

Please provide a two-week diary of your current practice, which you've completed within the last six months.
